 General Description

Continually engages consumers in activities designed to increase their independence. Must have knowledge of the training objectives being worked on and why the consumers receive prescribed training. Assists the consumer at the training facility that includes, but is not limited to, providing assistance on various trainings. Assist consumers with general hygiene care; safety/sanitation and communication. Reassignment is a condition of employment that may occur at the employer's discretion. Education, Training, and Experience (including licensure and certification)

Any combination of education or experience equivalent to high school graduation or GED. Valid driver’s license and insurable to drive Center owned vehicles and personal vehicle in performance of job related duties and activities; PIP Insurance required on personal vehicle. Must be able to participate in and successfully complete all required pre-service as well as other required trainings within 60 days of employment and as required thereafter.

Knowledge, Skills, and Abilities

Duties include, but are not limited to: continually engages in activities designed to increase their independence. Provide general custodial maintenance duties according to schedules. Interceding in behavior problems when needed. Follows Center policies and procedures and training objectives to provide safest environment possible for consumers being served in facility.

 General Description

This position will serve as clerical support for the clinical staff at the 500 Broad Street location. Responsibilities include, but are not limited to: Greeting and directing consumers to appropriate staff members, answering phone calls, directing calls to appropriate staff members or taking messages, correspondence, scheduling appointments, process mail and serve as back-up to other clerical support as needed. Position will perform complex data entry and run reports from various computer programs. Position requires initiative, excellent communication and organizational skills, and the ability to interact in a professional manner with a variety of staff members, consumers, family members, and community agencies. Work is performed with minimal supervision within established guidelines, policies, and procedures. Must attend all appropriate meetings. Education, Training, and Experience (including licensure and certification)

Graduation from standard high school or GED. Must possess working knowledge of fax machine, personal computer, and copier. Experience with Microsoft software, Microsoft Word, and Microsoft Excel is required. Minimum typing speed of 30wpm. Must have valid driver’s license and be insurable to drive Center vehicles. Must pass all Center mandated trainings.

Knowledge, Skills, and Abilities

• Ability to read and proof correspondence, reports, computer printouts, etc. • Excellent spelling and grammar. Ability to write memos, letters, various complex reports, miscellaneous documents. • Ability to perform addition and subtraction. Ability to count money. • Ability to complete financial assessments following procedure set out by TDMHMR. • Ability to relate to verbal instructions, communicate in a courteous and effective fashion with co-workers, consumers and families, agencies and the public. • Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office 95 and 97), CMHC, familiarity with CARE. • Ability to use copy machine, fax machine, computer and printer.  General Description

The Lead Houseparent works under the supervision of the Program Manager. The Lead Houseparent is responsible for the daily operation of the group home and has the authority to perform functions of supervising, training, transporting and caring for assigned individuals in compliance with ICF/MR regulations. Responsibilities include, but are not limited to: Completing staff work schedules, supervising staff in the group home, insuring adequate staff coverage, monitoring of consumer program implementation, consumer medication, monthly paperwork and other paperwork as needed. The Lead Houseparent also schedules medical appointments for consumers, attends Interdisciplinary Team (IDT) meetings and assists in the development of consumer’s Person Directed Plan (PDP). The Lead Houseparent is responsible for facilitating outcomes for people with disabilities in accordance with the Quality Assurance Improvement System (QAIS).  General Description

Under the direction of the Residential Coordinator, the Houseparent is responsible for and has the authority to perform the daily functions of supervising, training, transporting and caring for individuals in compliance with HCS principles to individuals living in their natural homes. Responsibilities include, but are not limited to: The supervision of residential management and support activities, individual training programs as defined in PDP and medication management ie. administration and supply to include environmental and safety functions. They will assist with the residential supports, training/PDP and safety reports. The Houseparent works rotating shifts with variable hours and participates in the residential on-call system. The Houseparent is responsible for facilitating outcomes for people with disabilities in accordance with Quality Assurance and Improvement System. Physical demands are moderate to heavy. The Houseparent maintains a cooperative relationship with a wide range of staff and community agencies. Reassignment is a condition of employment that may occur at the employer’s discretion.
