 | General Description |
Continually engages individuals with intellectual developmental disabilities in activities designed to increase their independence. Must have knowledge of training objectives being worked on and why the consumers receive prescribed training. Assist consumers with general hygiene care, safety, sanitation and communication. The Houseparent must be willing to work some rotating schedules. The Houseparent is responsible for facilitating outcomes for people with disabilities in accordance with the PDP.
Responsibilities include, but are not limited to:
Continually engaging in active treatment designed to increase consumer’s independence. Records pertinent data regarding consumer’s training objectives, health, problem behaviors and significant events. Transporting consumers and assisting with loading and unloading onto or from van. Provide general custodial maintenance duties according to schedules. Follows Center policies and procedures and training objectives to provide safest environment possible for consumers being served in facility. Staff will review Special Needs Notebook and keep informed of any new/special needs of consumers. Upon completion of assigned job duties, staff will initial by the assigned task in the assigned duties rolodex. Reassignment is a condition of employment that may occur at the employer’s discretion. Your work location and hours could change based on program needs. |

licensure and certification) |
Any combination of experience or experience equivalent to high school graduation or GED.
Must have valid driver’s license and be insurable to drive Center vehicles. Must have proof of PIP insurance. Must pass all center-mandated trainings. |

The Day Habilitation Trainer works under the general supervision of the Day Habilitation Coordinator (DHC). The employee is responsible for facilitating outcomes for people with disabilities.
Responsibilities include, but are not limited to:
This individual is responsible for and has the ability to provide day habilitation to individuals with mental and physical disabilities, speech impairment, in one or more phases of habilitation or vocational services, instruction, supervision, documentation, record keeping, safety, transportation and quality control. Some overnight travel may be required. The trainer will ensure compliance with HCS Principles, ICF/IDD regulations. The employee is responsible for facilitating outcomes for people with disabilities. High School graduate/GED, plus six (6) months experience in positions related to the duties of the position. Must have current driver’s license and be insurable under Center insurance. Position requires proof of PIP insurance. Must pass all Center mandated trainings. |

Assists in the development of and follows Individual Program Plans/Individual Plans of Care/individual Service Plans through formal training activities and individual supervision. Ensures availability to meet staffing needs of individuals. Utilizes meaningful, written and oral communication, and completes record keeping tasks. Ensures that medications are ordered, picked up, counted, and taken as prescribed. Monitors health and safety of individuals served and follows safety and infection control guidelines. Provides assistance for individuals who cannot manage their personal care needs. Transports assigned individuals and medications; performs and teaches housekeeping, home management duties and yard maintenance; assists with functional living and leisure skills, social skills, and money management; behavior intervention, physical assistance and other marginal duties as assigned. |
