Brief Description of Services
Eligibility Determination
An assessment or endorsement conducted to determine if
an individual has mental retardation or is a member of the TDMHMR
mental retardation priority population.
Service Coordination
This service is provided to individuals who have been
assessed and have multiple needs. It provides assistance in accessing
medical, social, educational and other appropriate services and
supports to help an individual achieve quality of life and community
participation acceptable to that individual.
Continuity of Services
Coordination of services for an individual residing in
a state facility whose movement to the community is being planned or
for an individual who formerly resided in a state facility and is on
community placement status. Also, provides service coordination for
individuals enrolled in the HCS, HCS-O, or ICF/MR program to maintain
the individual’s placement or to develop another placement for that
individual.
Respite Services
Services which temporarily relieve the family or
primary caregiver of their responsibility. Out of home respite is
provided on an emergency basis, away from the individual’s home.
In-home respite funds are provided families to secure respite services
in the natural home.
Employment Assistance:
Support service provided temporarily to an individual who is seeking
employment in the community and is not receiving Vocational Training.
Individualized Competitive
Employment: Support service provided to
enable an individual to maintain employment with an employer that
directly compensates the individual and has no more than 3% of its
workforce (or 1 person if 3% is less than 1) being persons with mental
retardation.
In-Home & Family Support
A grant program that provides assistance to purchase
items/services that are above and beyond the scope of usual needs and
are necessitated by the person’s mental disability.
